ADMISSION ENTRANCE EXAMINATION
The College requires students to qualify for admission on the basis of entrance examination:
GUIDELINES:
1. Interview (if required) with the Guidance Counselor for examination arrangements.
2. Accomplishment and Submission of the application form and attached the necessary
documents.
Transferees
Form 138
Certificate of good moral character
Two 2″ x 2″ I.D. pictures
Freshmen
Honorable Dismissal
Scholastic Rating or Transcript of Records
Two 2″ x 2″ I.D. pictures
3. Payment of the entrance examination fee and scheduling of actual examination.
4. Take the examination at the assigned room.
5. Examination and application results maybe obtained after a week.
ADMISSION PROCEDURES FOR TRANSFEREES
1. Present transfer credentials to the Registration Office.
2. The Registrar will endorse to the Dean s Office for interview.
3. The Dean may refer to the Guidance Office for entrance examination.
4. Secure results from the Guidance Office at least two (2) weeks after taking the exam.
5. If qualified, secure Application for Admission from the Registrar s Office.
6. Fill out the Application for the Admission.
7. Submit all credentials to the Registrar s Office.
8. Secure enrolment permit and student number from the Registrar s Office.
9. Proceed to your College Dean s Office for enrolment.
FOREIGN STUDENTS
Foreign Students maybe admitted upon compliance with all requirements and evaluation of their records by the Registrar s Office in coordination with the office of the Dean.
a. Study permit and Certificate of Eligibility from the Commision on Higher Education
(CHED).
b. Transcript of Records.
ENROLLMENT PROCEDURES FOR FRESHMEN STUDENTS
1. Secure Registration Form with attached curriculum checklist intended to enroll from your
College Dean s Office.
2. Fill out accurately and completely the Regstration and copy the schedule of clearance
from your College Dean s Office.
3. Enroll in NSTP at the DMST Office.
4. Proceed to the Accounting Office for assessment of fees.
5. Proceed to the Cashier s Office for the payment of assessed fees.
6. Surrender to the concerned office the portion of the registration form intended for the
Registrar, Accounting and Dean s Office copy.
7. Keep the student s copy of the registration form. records or its equivalent.
c. Letter of Recommendation from officials of school last attended.
d. A photocopy of the Alien Certificate of Registration.
ADMISSION PROCEDURES FOR CROSS ENROLLEES
1. Check with the Dean s Office if the desired subject is offered during the term.
2. The Dean will endorse to the Registrar s Office for Admission.
3. Secure Application for Admission from the Registrar s Office.
4. Fill out the Application for Admission.
5. Submit all credentials to the Registrar s Office.
6. Secure Enrolment Permit and Student Number from the Registrar s Office.
7. Proceed to your College Dean s Office for enrolment.
